This study evaluated the effects of aerobic interventions, alone or combined with the Mediterranean diet, on reducing cardiometabolic risk factors and preventing metabolic syndrome. A systematic review of studies published between 2019 and 2024 was carried out, according to the PRISMA guidelines, analyzing glycemia, blood pressure, lipid profile, abdominal circumference and inflammatory biomarkers. The results indicated significant reductions in blood glucose levels (8-15%), blood pressure (5-12%), abdominal circumference (5-12%) and inflammatory biomarkers, with an increase in HDL (10-20%). High-intensity interval training (HIIT) had a greater short-term metabolic impact, while moderate exercise showed greater adherence and safety in at-risk populations. The association with the Mediterranean diet enhanced the benefits, highlighting the synergy between exercise and nutrition. It is concluded that aerobic interventions are effective in the prevention and management of metabolic syndrome, promoting cardiometabolic health, reducing the costs of complications and improving quality of life
