ABDOME AGUDO NA PRÁTICA CIRÚRGICA: CONTRIBUIÇÕES DA IMAGEM DIAGNÓSTICA PARA DECISÃO CLÍNICA E CONDUTA OPERATÓRIA

O abdome agudo é uma das principais causas de atendimento em serviços de urgência e exige decisões rápidas sobre a necessidade de intervenção cirúrgica. Este estudo teve como objetivo analisar as evidências científicas sobre a contribuição dos métodos de diagnóstico por imagem na avaliação do abdome agudo e sua influência na tomada de decisão clínica e na conduta cirúrgica. Trata-se de uma revisão integrativa da literatura, com busca na MEDLINE via PubMed, complementada por busca manual em listas de referências e nas bases SciELO e LILACS. Foram incluídos 12 estudos, entre ensaios clínicos randomizados, estudos de acurácia diagnóstica, coortes, estudos transversais e metanálises. Os resultados demonstraram que a tomografia computadorizada apresentou maior sensibilidade que a ultrassonografia para condições urgentes, modificou o diagnóstico principal em cerca de metade dos pacientes, alterou decisões de internação e de indicação cirúrgica e esteve associada à redução de apendicectomias negativas. A estratégia condicional, com ultrassonografia inicial e tomografia nos casos negativos ou inconclusivos, preservou a sensibilidade e reduziu a exposição à radiação. Conclui-se que a imagem diagnóstica qualifica a decisão cirúrgica no abdome agudo, desde que seu uso seja criterioso e integrado à avaliação clínica.
